    Provides case m    anagement services for the mentally ill and geriatric members of the community--using least restrictive methods including but not limited to DPOA-Trustee-Guardianship
    Fees: $45/hr billed by 1/10 of hour
    Eligibility: Geriatric person in need of case management through guardianship.
    Intake procedure: Anyone can call in referral to office and the staff will assess the consumer's needs.
